One possibility is Reversal of Fortune. Has happened to me with stone daggers in PvE a few times, mostly used on purpose to negate it. Since it doesn't show you whether or not you heal them now, it's easy to get confused.

For the Tomb Nightmare guy - Thats always happened. he has x4 weakness
to cold dmg.

Thats how you beat him (You are expected to be lvl1-2 when you do that quest..fighting solo vs. a lvl4..if it didnt have x4 weakness to cold you'd die in like 2 seconds).
